Despite everything I say here, this is a great little book and very evidence focused. Missing chapters that would be useful include chronic headache, FEMALE chronic pelvic pain (male is included, go figure).

The authors offer little information on the use of cannabis, perhaps because the book went to press in 2006 and several RCTs have be published since giving us a better understanding of is risks and benefits. As a palliative care provider, I would appreciate some information on this topic.

I was interested that the authors statement there is no clearly superior fractionation schedule for palliative radiation therapy in cancer pain. Faced with similar outcomes between single and multi-faction therapy near end-of-life when burdens of care are so crucial, single fraction therapy is a very logical choice, though often discounted by radiation oncologists. We need clear statements by experts that single fraction therapy near EOL is preferable.

In all though, this book is a very good buy and primary care providers should keep it handy!
